A Registered Pharmacist who fails to renew his/her Registration before 31st March of the concerned year, the name of such Pharmacist is removed from the Register in accordance to the provisions of the Pharmacy Act, 1948 and the rules there under. If the Pharmacist desires to re-enter his/her name in the Register, he/she has to apply in a prescribed Form-L along with documents and prescribed fees.

List of Documents Required

1. Re-Entry Application Form, Form-L

2. Forwarding Letter

3. Self attested copy of Pharmacist Registration Certificate

4. Self attested copy of Highest Degree Certificate as entered in the Register.

5. Proof of Employment/Business of the candidate [(a) Employer’s Certificate mentioning period of service, designation, head quarters etc. from relevant Firm/Company/Institution mentioning Drugs License No’s, if any ; or (b) In case of self employed persons, A self declaration on the letter pad of the relevant Firm/Company etc. mentioning Drug License No.s if any;

6. Witness information of existing Registered Pharmacist in Form-L with complete name, address, registration number and contact details

7. Two (02) Recent Passport Size Photograph

8. Copy of Aadhar Card

9. Self Addressed Envelope with postage stamp of Rs 30/-
