CHANGE IN THE REGISTERED NAME

A Registered Pharmacist who has changed his/her name or a part of his/her name, he/she should submit an application in prescribed form along with documentation of change or alteration in any part of the name and fees of Rs. 200/- (Rupees Two hundred Only)

List of Documents Required

1. Application for Change of Name

2. Form for Change of Name

3. Marriage certificate (In the case of married female candidate) or a copy of gazette notification (in all other cases)

4. Any one address proof in New Name
