TRANSFER OF REGISTRATION

(a) Transfer to Tripura

A person who is a Registered Pharmacist in other State Pharmacy Council and is now residing in Tripura and intending to carry out the business or profession of Pharmacy in Tripura, can apply for the transfer of his/her registration to the Tripura State Pharmacy Council by submitting an application in prescribed Form-G with necessary supporting documents, testimonials and whatever information required by this Council along with prescribed fee.

The permission for the Transfer of Registration to Tripura shall be subject to the applicant’s qualification for Registration under the provision of Pharmacy Act, 1948 and rules thereof. Moreover, State Pharmacy Council need to receive “No Objection Certificate” and relevant documents and information’s from the concerned State Council where the Pharmacist was originally registered.

1. Application to the Registrar for the Transfer of Registration.
2. Original Registration Certificate of concerned State Pharmacy Council & Renewal Receipt of having the Registration renewed up to -date
3. Form-G
4. Forwarding Letter (Included in Form-G)
5. Declaration as Appendix-1
6. Code of Pharmacy Ethics duly signed by the Candidate
7. Pharmacist’s oath duly signed by the Candidate
8. Three recent passport size photograph
9. Proof of Birth Date & Birth Place
10. Aadhar Card
11. Mark Sheet of Madhyamik Pariksha (Secondary Examination)
12. Certificate of Madhyamik Pariksha (Secondary Examination)
13. Mark Sheet of Higher Secondary Examination (+2) Science
14. Certificate of Higher Secondary Examination (+2) Science
15. Degree/Diploma in Pharmacy Mark sheet of all years and all semesters
16. Degree/Diploma in Pharmacy Certificate obtained from relevant University/Board of examination
17. Practical Training Completion Certificate (Hospital/Industry)
18. Self declaration of non-practicing (Annexure-B).
19. An affidavit (as per specimen copy enclosed in the case of applicant who has passed Diploma / Degree in Pharmacy examination from an Institution of Tripura or other than Tripura State) (Annexure-C)
20. An affidavit (as per specimen copy enclosed in the case of applicant who has been registered as Pharmacist in the State Pharmacy council other than Tripura) (Annexure-D)

(b) Transfer from Tripura

The Pharmacist who wants to transfer the registration from Tripura State Pharmacy Council to another State Pharmacy Council, he/she has to surrender Original Registration Certificate and Original Renewal Receipt to the concerned State Pharmacy Council.

It is to be noted that for the transfer of registration, Pharmacists Registration must be valid upto current year with Tripura State Pharmacy Council. If it is not valid upto current year, kindly renew / re-entry before transfer of registration.
