A Registered Pharmacist is eligible to get duplicate copy of their Pharmacy Registration Certificate under the following circumstances.

1. The Certificate is irrecoverably lost, or
2. The certificate is damaged

In the case of damage, the Registered Pharmacist will submit an application (Specimen as per Annexure-F), stating the fact along with the damaged certificate and other documents as required. In case the certificate is lost, an affidavit (Specimen as per Annexure-G) on the stamp paper worth Rs.20/- (Rupees Twenty only) and signed by a judicial officer, not below the rank of 1st class Judicial Magistrate, should be submitted. The prescribed fee for the duplicate certificate should also be submitted.

List of Documents Required

1. Application for Duplicate Registration Certificate (Annexure- F)

2. Affidavit for Duplicate Registration Certificate (Annexure-G)

3. Copy of the General Diary (GD) with GD entry number, seal with a sign for the lost Registration Certificate filed in the Police station.

4. Address Proof (Any one from (a) Voter Identity Card of the Candidate issued by the Election Commission (b) Aadhar Card of the candidate, or (c) Passport of the Candidate) and Permanent Residence Certificate of Tripura (PRTC).
